- Please complete the appropriate UR form and submit your application materials by the deadline printed to ensure timely processing and full consideration.
Returning undergraduate students who do not submit their financial aid application with all supporting documentation by the April deadline should be aware that they are not guaranteed full consideration for need-based assistance and may have their aid reduced due to limited funding. - Aid Request Form
- If you would like to request a change to your financial aid award, please complete this form and return it to our office. Financial Aid Request 2007-08
- Federal Direct PLUS Application
- Parents of undergraduate students can complete this application to apply for a PLUS. Please note that this form requires a parent signature. PLUS Application 2008-09
- Parent PLUS Action Form
- Parents who have already applied and been approved for the Parent PLUS loan may make adjustments to the award amounts. Please return this form to our office for review. PLUS Action Form 2008-09
- Federal Direct Graduate PLUS Application
- Graduate students can apply for a Federal Direct Graduate PLUS loan. Borrowers must also receive a Federal Direct student loan in order to qualify. Grad PLUS Application 2008-09
- Consortium Agreement
- A consortium agreement is required each time a student registers for credit at another institution while a matriculated student at UR and receives financial assistance. University assistance cannot be used for courses outside UR; the most common type of assistance processed under a consortium agreement is loans. Consortium Agreement 2008-09
- Graduate PLUS Action Form

- Summer Aid Request Form
- Undergraduate students enrolling in summer courses who are interested in alternative loans to cover the costs of these courses must complete the Summer Aid Request Form and submit it to the Financial Aid Office. Summer Aid Request Form 2008-09
